How To Apply

Step 1

Fill in the online form below and receive a confirmation email; OR

Request for a registration form at any of the hospitals, fill in the details, submit to the Customer Care personnel and receive a temporary card.

Step 2

Visit the hospital and show us the confirmation email / temporary card together with your child’s birth certificate or IC

* RM20 registration fee applies, if the child is neither a sibling of a Columbia Kids Club (CKC) member nor born at Columbia Asia hospitals. Sibling of CKC member is entitled to 50% discount on registration fee.

Step 3

Collect the Permanent Card and ONE (1) Zoey Plush toy

Step 4

Enjoy the privileges as a CKC member
